We are seeking a skilled and experienced Registered Nurse to join our team as a Transfer Center RN at Delray Medical Center. As a key member of our healthcare team, you will play a vital role in coordinating direct admissions and hospital transfers to ensure seamless patient care.
Responsibilities- Collaborate with referring facilities, physicians, and hospital staff to foster strong relationships and represent our organization's mission and values.
- Work closely with clinical, medical staff, and leadership teams to identify and report barriers to patient access and throughput.
- Utilize strong problem-solving skills to resolve issues and ensure efficient patient flow.
- Develop and maintain effective communication with patients, families, and healthcare teams to ensure high-quality care.
- Stay up-to-date with industry developments and best practices to continuously improve patient outcomes.
- Minimum of four (4) years of clinical experience in an acute care setting, including Charge Nurse, House Supervisor, or related management experience.
- Registered Nurse licensure in the State of Florida.
- Strong leadership skills, with the ability to handle multiple tasks and remain flexible.
- Proficiency in EMR's, Word Processing, and Excel spreadsheets.
- Excellent communication and interpersonal skills, with the ability to interact with patients, families, and healthcare teams in a professional and compassionate manner.
