Position Overview:
The Practical Nursing Specialist is responsible for delivering nursing care, treatments, and procedures to clients in their homes, adhering to the guidelines of their training, state licensure, and agency policies. This role involves executing tasks as directed by the Case Manager or Clinical Team Manager.
Key Responsibilities:
- Assist the Registered Nurse or Physician in executing specialized procedures and responsibilities.
- Deliver prescribed nursing care procedures to designated clients.
- Engage in assessing, planning, implementing, and evaluating client care in partnership with an RN.
- Provide health education and support clients and families in mastering self-care techniques.
- Document findings in a clear, concise, and timely manner in accordance with agency policies.
- Record services provided, client conditions, and responses to treatments.
- Adhere to safety protocols in compliance with OSHA, CHAP, and federal and state regulations.
- Prepare clinical and progress notes accurately.
- Utilize medical equipment correctly for client care.
- Exhibit knowledge and skills necessary for delivering appropriate client care.
- Promptly carry out medical orders received.
- Identify and communicate changes in client conditions or potential issues in a timely manner, following the proper chain of command.
- Practice good hand hygiene and aseptic techniques.
- Perform client care duties competently and safely, including administering medications and executing procedures as per agency policy.
- Demonstrate understanding of growth and development principles across the lifespan.
- Participate in the Performance Improvement program by utilizing feedback mechanisms and addressing concerns with management.
- Keep management informed about potential issues or concerns and consult with clinicians or managers as necessary.
- Maintain awareness of the company's service offerings.
- Engage in interdisciplinary meetings as required.
- Submit clinical documentation promptly in line with agency policies.
- Uphold a professional appearance.
- Employ appropriate telephone etiquette.
- Foster a welcoming atmosphere for all visitors while ensuring client confidentiality at all times.
- Partake in the on-call schedule as needed by the agency.
- Contribute to the accreditation program.
- Perform additional duties as assigned.
- Engage in in-service training programs.
Required Skills:

Education and Experience:
Graduation from an accredited LPN program is required, along with a minimum of one year of nursing experience. Home care experience is preferred.
Licenses and Certifications:
Current LPN license in the state of Florida, CPR certification, valid driver's license, and auto insurance. Completion of 25 CEUs bi-annually is required.
Work Environment:
Office/Home setting. May involve exposure to biological hazards.
