**Job Summary**
The MDS Coordinator and the Registered Nurse Assessment Coordinator (RNAC) play a crucial role in the coordination of patient/resident care from admission through discharge at Las Brisas Rehabilitation and Wellness Suites. Under the direction of the Director of Nursing (DON), this position is responsible for ensuring seamless care transitions and accurate documentation.
**Key Responsibilities:**
- Identify and monitor resident skilled services to ensure timely and effective interventions.
- Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and comprehensive assessments.
- Complete accurate Minimum Data Set (MDS) and develop interdisciplinary Plans of Care that meet resident needs.
- Monitor patient/resident status to identify changes in condition, barriers to discharge, and progress toward care goals.
- Participate in daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meetings to ensure collaborative care planning.
- Establish and maintain positive relationships with healthcare team members to facilitate effective communication and care coordination.
**Requirements:**
- Current, unencumbered license in the state of practice.
- Minimum of 2 years of direct hands-on nursing experience.
- Minimum of 2 years of long-term care experience.
- Preferably 1 year of RAI experience.
- Proficiency in ICD-10 coding.
