Key Responsibilities:
- Assist in the development, review, and revision of care plans.
- Deliver nursing care in accordance with the physician's treatment plan and individual care plan, as authorized by the client's payer source.
- Monitor, document, and report any reactions to treatment and changes in the client's condition to the appropriate personnel.
- Provide ongoing instruction and supervision to clients, families, or other healthcare team members regarding the treatment plan.
- Record and monitor the patient's condition and document care services using electronic charting.
- Participate in educational programs, quality assessment activities, and staff meetings as required.
Qualifications:
- Active Licensed Practical Nurse or Registered Nurse License
- Strong problem-solving skills and ability to manage multiple tasks
- Excellent communication abilities
