Key Responsibilities:

  • Assist in the development, review, and revision of care plans.
  • Deliver nursing care in accordance with the physician's treatment plan and individual care plan, as authorized by the client's payer source.
  • Monitor, document, and report any reactions to treatment and changes in the client's condition to the appropriate personnel.
  • Provide ongoing instruction and supervision to clients, families, or other healthcare team members regarding the treatment plan.
  • Record and monitor the patient's condition and document care services using electronic charting.
  • Participate in educational programs, quality assessment activities, and staff meetings as required.

Qualifications:

  • Active Licensed Practical Nurse or Registered Nurse License
  • Strong problem-solving skills and ability to manage multiple tasks
  • Excellent communication abilities
