Job Overview: The Staff Clinical Pharmacist is tasked with delivering patient-focused pharmaceutical care while managing traditional drug distribution and control operations. This role encompasses providing clinical consultations, including the precise dosing of antibiotics such as vancomycin and aminoglycosides, initial warfarin dosing, renal dosing adjustments, and transitions from IV to oral medications. Exceptional clinical acumen and communication skills are essential for delivering drug-related information to nursing staff, physicians, and other healthcare professionals. The pharmacist will also guide the medical team on drug interactions, contraindications, and other critical information to ensure safe and effective medication therapy.
Key Responsibilities:
- Assessing drug appropriateness by evaluating and monitoring medication regimens.
- Interpreting physician orders and screening for potential interactions.
- Providing education to patients and nursing staff as needed.
- Reviewing medication profiles to ensure accuracy and optimize therapy.
- Performing all duties associated with a Pharmaceutical Care Pharmacist.
- Preparing IV admixtures, compounding, and repackaging a variety of pharmaceuticals.
- Entering and verifying orders in the pharmacy information system, ensuring accurate medication dispensing.
- Monitoring patients for antibiotic and anticoagulant dosing.
- Participating in multidisciplinary rounds.
- Collaborating with healthcare providers to educate patients about new medications and their potential side effects.
- Assisting with medication reconciliation and providing discharge counseling.
Licensure and Certification: All necessary certifications must be obtained upon hire unless otherwise specified.
- Registered Pharmacist
Experience Requirements: Previous experience in a hospital pharmacy setting is preferred, with a minimum of two years of clinical pharmacy experience also preferred.
Skills Required: Proficient order entry and computer skills, strong verbal communication abilities, independent critical thinking, and the capacity to collaborate effectively with other professionals to enhance patient care.
