We are seeking a skilled Home Infusion Nurse to join our team at Amerita. As a Home Infusion Nurse, you will provide professional nursing care for the safety and well-being of our patients in the comfort of their own homes.
Responsibilities- Assess patients and develop individualized plans of care to meet their unique needs.
- Administer medications and therapies as prescribed by physicians, ensuring accurate dosing and monitoring for potential side effects.
- Collaborate with pharmacy staff, physicians, and patients to develop and monitor treatment plans.
- Provide education and support to patients and their caregivers on medication management, infusion techniques, and other relevant topics.
- Conduct regular assessments and evaluations to ensure patients are meeting their treatment goals.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Graduate of an accredited school of nursing with a valid nursing license in the state(s) of practice.
- Minimum of one year of experience as a Registered Nurse, with a focus on infusion therapy and home health care.
- Current CPR certification and a valid driver's license with a clean driving record.
- Ability to work effectively within a multidisciplinary team and communicate effectively with patients, families, and healthcare teams.
