Your Responsibilities

  • Perform personalized in-home therapy sessions, developing tailored treatment plans that deliver real results.
  • Guide patients in improving strength, balance, and mobility, reducing fall risks, and enhancing independence.
  • Educate patients and families on safe daily living activities, such as using assistive devices and preventing falls.
  • Document treatment progress, patient status, and outcomes using the WellSky/Kinnser EMR system.
  • Collaborate with physicians and case managers to deliver seamless care.

What We're Looking For

  • A passionate and self-motivated professional who holds a valid California PT license.
  • A professional with AHA CPR certification.
  • A reliable, organized, and personable individual.
  • An experienced professional with a minimum of one year of experience (Home Health experience is a plus).
  • An individual with reliable transportation, a valid driver's license, and auto insurance.
  • A professional with excellent communication and problem-solving skills.
