The PACE (Program of All-Inclusive Care for the Elderly) model was created to address the numerous challenges associated with caring for vulnerable seniors. This approach emphasizes the importance of providing care within the community, ensuring the well-being of seniors with chronic health needs and their families.

Participants, along with their families and caregivers, often encounter various challenges, such as transportation to medical appointments, medication management, coordination of care among multiple specialists, social isolation, and the ability to live independently at home. Through On Lok PACE, participants benefit from in-home care services and transportation to a PACE center for primary healthcare, social engagement, and additional senior services.

POSITION OVERVIEW: Under the administrative guidance of the Supervising Physician, the Staff Physician is accountable for the medical care of participants enrolled in the On Lok program.

KEY RESPONSIBILITIES:

1. Conduct comprehensive medical histories and thorough physical examinations for all prospective clients and new enrollees in the On Lok program.

2. Collaborate with the multidisciplinary Intake and Assessment Team to develop tailored treatment plans for each new enrollee.

3. Monitor and document medical changes, performing interval physical examinations on participants according to established policies, with a minimum requirement of every six months.

4. Act as the primary care physician for participants, addressing acute illnesses in various settings, including clinics, hospitals, nursing homes, and participants' homes.

5. Provide case management advice and collaborate with Nurse Practitioners for consultations as needed.

6. Work closely with the Clinic Nursing Staff to ensure coordinated care.

7. Request and authorize consultations from On Lok's network of medical and surgical specialists when necessary.

8. Collaborate with in-house medical specialists, including Dentists, Podiatrists, Audiologists, and Speech Therapists.

9. Serve as the primary physician or consult with the Hospitalist for participants admitted to acute care hospitals.

10. Deliver primary care for participants residing in nursing homes, with monthly examinations.

11. Provide on-call coverage for direct patient care on a rotational basis with other On Lok staff physicians as required.

12. Complete all administrative responsibilities promptly, including accurate record-keeping in participant charts and participation in staff meetings.

13. Offer in-service training as necessary.

14. Represent the On Lok program to interested groups and individuals.

15. Perform other related duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities):

1. M.D. degree and a current, valid license to practice medicine and surgery in California.

2. Formal training in Internal Medicine or Family Practice; Board eligible or Board Certified in either specialty.

3. Subspecialty training or a minimum of one year of experience working with elderly or frail populations is required.

4. Current and valid Drug Enforcement Agency Certificate.

5. Certification in Basic Life Support or Advanced Cardiac Life Support.

6. Willingness to collaborate within a multidisciplinary team in the On Lok health delivery system.

7. Sensitivity to the health beliefs and needs of a diverse elderly population.

8. Strong communication and leadership skills.

9. Bilingual abilities may be required for specific roles.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
