Job Description

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Care Coordinator. In this role, you will provide vital in-home nursing care for former Nuclear Weapons Workers dealing with chronic illnesses due to toxic exposure.

About the Position:

  • You will be responsible for conducting physical assessments, overseeing caregivers, and ensuring the safety and well-being of clients in their homes.
  • You will manage case delivery with a team of RNs, LPNs, and caregivers, ensuring high-quality care and adherence to agency policies.
Responsibilities
  1. Conduct weekly in-home nursing visits for assigned clients, typically lasting 1-2 hours per visit.
  2. Perform physical assessments, including vitals, and ensure home safety with medical equipment and supplies.
  3. Oversee skilled and non-skilled caregivers providing care in the client's home.
  4. Provide direct care and case management of assigned clients.
  5. Assist and collaborate with regional directors and other personnel to identify and correct issues and/or improve services.
Requirements
  • Graduate of a state-approved school of professional registered nursing.
  • BSN preferred.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
  • Current CPR certification.
