We are seeking a dedicated Psychosocial Assessment Coordinator to join our Axess Family Services team. As a key member of our behavioral health team, you will play a vital role in assessing client needs and developing effective treatment plans.

Main Responsibilities
  • Conduct thorough psychosocial assessments to identify client needs and develop tailored treatment plans.
  • Collaborate with healthcare teams to provide coordinated care, focusing on mental health conditions and substance abuse.
  • Work closely with community-based programs and agencies to expand our network and improve client outcomes.

Requirements

  • A Master's degree in Behavioral Health, Social Work, Counseling, or a related field.
  • Working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and colleagues.

We offer a competitive salary range of $60,000 - $80,000 per year, commensurate with experience, plus excellent benefits and opportunities for professional growth.
