About the Role

As a Director of Patient Care, you will be responsible for leading our Hospice Home Care Transdisciplinary Teams, ensuring the delivery of comprehensive, coordinated quality clinical care and service in homes.

Key Responsibilities
  • Coordinate clinical staff to provide high-quality patient care.
  • Evaluate facilitators of Care Conferences for Hospice Care Teams and provide guidance to Team Leaders.
  • Supervise and develop Team Leaders to achieve excellent patient outcomes.
  • Maintain open communication with all hospice community offices and coordinators.
Requirements
  • Five (5) year's experience in hospice home care management.
  • Assertive and diplomatic in relating to people.
  • Systematic and organized.
  • Knowledgeable in hospice home care and community relations.

The estimated salary for this position is $85,000 - $110,000 per year, depending on qualifications and location.
