Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to improve their overall health and well-being.

Responsibilities
  • Conduct thorough health assessments in patients' homes, including medical history, physical examination, and review of medications
  • Identify potential health risks and develop personalized care plans to address these risks
  • Collaborate with patients, families, and healthcare providers to implement care plans and improve health outcomes
  • Provide education and support to patients and families to promote healthy behaviors and self-management of chronic conditions
  • Document all assessments and care plans in accordance with Matrix Medical Network's policies and procedures
Requirements
  • Masters degree in nursing or a related field, or equivalent experience and licensure
  • Current RN and NP or PA licensure in the state of practice, with prescription authority or ability to obtain
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
