About the RoleWe are seeking an experienced Ambulatory Cardiovascular Nurse Specialist to join our team at Mass General Brigham. As a key member of our nursing staff, you will provide high-quality patient care and work collaboratively with our healthcare team to deliver exceptional patient outcomes.
Job SummaryThe Ambulatory Cardiovascular Nurse Specialist will be responsible for providing telephone triage, collaboration with Watkins Cardiology Access Center, clinic, and arrhythmia services ambulatory RN for cross-training and coverage as needed, communication with patients via electronic health information exchange Patient Gateway/MyChart, phone, fax, mail, and email as necessary, gathering patient data and follow-up on requests/review consults with MD/NP to develop care plan, completing post-hospital discharge phone calls including medication reconciliation, education of patients and family regarding cardiovascular/arrhythmia disease and management to patients telephonically and in the ambulatory setting, assisting with management and maintenance of the remote monitoring program, maintaining communication with device company personnel and MD/NP regarding specific device issues, assisting with management of patient database including return intervals, missed appointments and active/inactive list, performing post-procedure wound checks as indicated, setting up consults with specialty providers, follows up on consult recommendations, and communicates results to patient and providers, assistant with communication and correspondence to/from patients, employers, pharmacies, and insurance providers, willingness to cross train to perform cardiac device interrogation for implanted diagnostic loop recorders, pacemakers, internal cardiac defibrillators, and heart failure resynchronization devices based on established guidelines, allowing the ability to cross-cover device clinics on an as needed basis, and performs all other duties as required or assigned within the scope of the position.
Responsibilities- Provide telephone triage for patients with implanted cardiac devices.
- Collaborate with Watkins Cardiology Access Center, clinic, and arrhythmia services ambulatory RN for cross-training and coverage as needed.
- Communicate with patients via electronic health information exchange Patient Gateway/MyChart, phone, fax, mail, and email as necessary.
- Gather patient data and follow-up on requests/review consults with MD/NP to develop care plan.
- Complete post-hospital discharge phone calls including medication reconciliation.
- Education of patients and family regarding cardiovascular/arrhythmia disease and management to patients telephonically and in the ambulatory setting.
- Assist with management and maintenance of the remote monitoring program.
