We are seeking a highly skilled and compassionate Home Health Nurse to join our team at Stonerise Home Health South Charleston. As a Home Health Nurse, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.
Key Responsibilities- Conduct thorough assessments of patients to determine their care needs and develop personalized care plans.
- Supervise and direct the implementation of care plans by Licensed Practical Nurses (LPNs) and Home Health Aides.
- Develop and implement patient education plans to promote health and wellness.
- Evaluate and revise care plans as patient needs and status change.
- Administer medications and treatments as prescribed by physicians.
- Identify discharge planning needs and coordinate patient care to ensure a smooth transition.
- Act as a Case Manager for assigned caseloads and coordinate patient care.
- Graduate of an accredited school of nursing.
- Active RN licensure.
- One to two years of recent acute care experience in an institutional setting.
- Bachelor's degree with one year of home health care experience preferred.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements.
