Responsibilities

The successful candidate will be responsible for providing comprehensive medical care to patients, including evaluation, assessment, diagnosis, and treatment. This may involve developing plans of care, prescribing medication and treatment, and collaborating with other healthcare professionals to ensure seamless care coordination. In addition, you will be expected to maintain accurate and up-to-date clinical records, communicate effectively with patients and their families, and participate in interdisciplinary team meetings to discuss patient care and progress. Other responsibilities may include making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care, and ensuring compliance with all relevant laws, regulations, and standards.

Requirements

To be considered for this role, you must have a graduate degree from an accredited School of Nursing and an accredited Nurse Practitioner program. You must also possess current DEA license, if applicable to state regulations, and current nursing license in the state of practice. Additionally, you should have a master's degree with minimum of one year NP experience, and one year hospice or palliative care experience is preferred. Accreditation from an approved certifying body for advanced practice nursing as required by state of practice is essential, and certification in a specialist area such as Hospice and Palliative Nursing (CHPN) or Geriatrics is highly valued. Finally, a commitment to clinical and documentation excellence is critical.
