Nurse Practitioner

Mass General Brigham Healthcare at Home is seeking a skilled Registered Nurse (RN) to provide high-quality clinical services to patients in various settings within the community. As a key member of our interdisciplinary team, you will deliver patient care that aligns with our organization's philosophy, policy, and standards of community health practice.
Responsibilities- Provide skilled nursing care to patients in various settings, including their homes, with a focus on assessment, teaching, treatment, documentation, and care coordination.
- Collaborate with other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
- Utilize electronic documentation and interdisciplinary communications to ensure timely and thorough documentation of patient care.
- Participate in the admissions process and case management responsibilities, adhering to MGB Healthcare at Home policies and standards of practice.
- Work effectively with patients, families, and caregivers to promote patient-centered care and achieve optimal outcomes.
- Graduate of an approved School of Nursing.
- Current Massachusetts license as Registered Nurse.
- Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred.
- Ability to travel within the MGB Healthcare at Home geographic area.
- Strong interpersonal and customer service skills, with the ability to work with diverse populations.
- Effective communication skills, both written and verbal, with the ability to work as a member of an interdisciplinary team.
- Ability to organize and prioritize work, adapt to changing situations, and work independently with minimal supervision.
