Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a key member of our interdisciplinary team, you will provide direct care and support to patients and their families, ensuring a seamless and high-quality care experience.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the healthcare team to ensure timely and effective care coordination, including medication management and treatment administration.
- Provide emotional support and education to patients and their families, empowering them to make informed decisions about their care.
- Maintain accurate and detailed clinical records, ensuring compliance with regulatory requirements.
- Participate in interdisciplinary team meetings to discuss patient progress and develop strategies for optimal care.
- Graduate of an accredited nursing program with current licensure to practice in the state of operation.
- At least one year of nursing experience, with a preference for experience in hospice or a similar setting.
- Knowledge of the hospice philosophy of care and commitment to clinical excellence.
