About the RoleAs an MDS Nurse, you will work closely with our social services team to provide high-quality care and referral services to our residents. You will be responsible for conducting assessments and care plan coordination, completing the MDS, CAA's, and care plans within regulated time frames, and reviewing information from hospitals, consults, and outside agencies.
Key Responsibilities- Conduct physical assessments, interviews, and chart reviews to identify resident care needs
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
- Review information from hospitals, consults, and outside agencies and use such information in the completion of the assessment and care planning
- Prepare for and conduct PPS meetings, maintaining MDS assessments per Medicare schedule and monitoring Medicare days and RUGs utilization
- Remain current with the American Association of Nursing Assessment Coordinators (AANAC) requirements
