Job Summary
PIH Health is seeking a skilled Occupational Therapist I, Home Health to provide high-quality patient care and support in the comfort of their own homes. As a key member of our home health team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized therapy plans that promote independence, mobility, and overall well-being.
Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and cognitive abilities to identify areas of need and develop targeted therapy plans.
- Design and implement individualized therapy programs to improve patients' functional abilities, mobility, and daily living skills.
- Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and communication.
- Provide education and training to patients and caregivers on proper techniques, safety measures, and adaptive strategies to promote independence and confidence.
- Document patient progress, outcomes, and any changes in condition to ensure accurate and timely reporting.
- Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain expertise and provide high-quality care.
Requirements
- Current licensure as an Occupational Therapist in the state of California.
- Graduate of an accredited occupational therapy program.
- Minimum of two years' experience in home health or a related field.
- Basic computer knowledge and skills, including proficiency in electronic medical records and other relevant software.
- CPR certification and valid California driver's license.
- Valid automobile insurance.
Preferred Qualifications
- Experience working with geriatric or pediatric populations.
- Knowledge of home health standards and practices, including assessments and documentation requirements.
- Ability to work independently and as part of a team to achieve patient-centered outcomes.
