Job Summary

As a Social Worker at LifeBridge Health, you will play a vital role in providing direct services to patients and families to identify and resolve problems that impede discharge planning or the patient's use of healthcare resources.

Key Responsibilities
  • Provide assessment and discharge planning to adult and geriatric populations
  • Document all interventions and services provided
  • Collaborate with physicians, nursing, and other members of the multidisciplinary team to promote care coordination
  • Intervene with patients and families regarding emotional, social, and financial consequences of illness and/or disability
  • Access and mobilize family/community resources to meet identified needs
Requirements
  • Master's Degree in Social Work/Counseling from an accredited institution
  • Current licensure in the State of Maryland – LMSW/LGPC required; LCSW-C/LCPC preferred
  • Minimum of 1 year of experience in a healthcare setting; hospital preferred
