Nurse - Hospice Care Specialist

Responsibilities:- Assess patient needs and develop individualized care plans
- Partner with interdisciplinary teams to implement and manage plans of care
- Consult with patients and families regarding disease processes and self-care techniques
- Communicate with medical directors and clinicians regarding patient needs and outcomes
- Prepare and submit documentation of observations and services provided
