Overview:
The Care Navigation Nurse Practitioner plays a vital role in providing phone-based care management to patients and families throughout our continuum of care. This includes home and facility-based hospice, home and facility-based palliative care, inpatient hospice, hospital-based palliative care consultation, and outpatient palliative care clinics and Primary Care at Home.
As a key member of our interdisciplinary teams, the Care Navigation Nurse Practitioner provides clinical support to the Care Navigation team, Triage Staff, and Interdisciplinary Teams during evening and weekend hours to ensure timely communication of care management activity.
The successful candidate will develop methods to improve clinical outcomes, ensuring the continued quality of care and timely communication of evening and weekend phone-based care management activity.
Responsibilities:
- Advocate for patients by translating complex medical information into language they can understand.
- Develop and implement care plans to ensure seamless transitions of care.
- Collaborate with interdisciplinary teams to ensure timely communication of care management activity.
- Identify opportunities to improve clinical outcomes and develop methods to address these gaps.
Qualifications:
- Must have 5 years of recent clinical experience, including experience in hospice, palliative care, and/or acute inpatient care.
- Must have a Master's degree in Nursing and completed an accredited Nurse Practitioner program.
- Must have medical licensure from the governing body for medical practice for the District of Columbia, State of Maryland, and the Commonwealth of Virginia.
- Must maintain medical licensure, prescriptive authority, and DEA licensure for all Capital Caring Health jurisdictions.
