Under the direct oversight of the designated Wexford Health manager and clinical guidance from the Director of Nursing, Registered Nurses, Medical Director, and/or appropriate personnel, the Licensed Practical Nurse (LPN) is tasked with facilitating patient care through the nursing process, which includes assessment, planning, implementation, and evaluation. The LPN will also guide patient education activities in alignment with their qualifications and demonstrated competencies.
The Licensed Practical Nurse will deliver a variety of services to the center, encompassing assessment, medication administration, non-professional nursing tasks, and support for Registered Nurses and medical staff.
Under the supervision of a Physician or Registered Nurse, the LPN will perform non-professional nursing duties, assessing the physical, psychological, and social aspects of patients within the Health Care Unit and, when necessary, in the housing units.
The LPN will assist in the development of individualized treatment plans by utilizing available resources and collaborating with registered nurses, physicians, and correctional officers.
They will also aid in the execution of these treatment plans within the Health Care Unit and, as required, in the housing unit.
Key responsibilities include:
- Implementing clinical and technical care aspects in accordance with established policies and procedures.
- Administering medications and interventions as per physician directives.
- Documenting nursing encounters, patient responses, effectiveness, and complications, while communicating pertinent information to supervisory personnel and physicians.
- Observing, recording, and reporting symptoms, reactions, and changes in patient conditions.
- Utilizing available in-house resources for treatment and consultation.
- Respecting the dignity and confidentiality of all patients.
- Taking vital signs (temperature, pulse, respiration, and blood pressure).
- Applying and changing dressings.
- Ensuring the emotional and physical comfort and safety of patients.
- Assisting with daily living activities, emphasizing their therapeutic significance.
- Maintaining cleanliness and order in working and living areas.
- Participating as a member of the nursing care and treatment team.
- Assisting in the orientation of new employees.
- Supporting physicians as assigned.
- Supervising inmates in maintaining cleanliness and sanitation of the environment.
- Assessing personal and professional development needs.
- Planning and implementing programs to address identified needs through various training formats.
- Demonstrating knowledge of nursing policies and procedures.
- Upholding the philosophy, objectives, and goals of the nursing department.
All activities will be performed within the normal limits of the State Nursing Practice Act, Code of Ethics for Nurses, and facility policies and procedures.
Additionally, the LPN will maintain the confidentiality of inmate records, approach changes positively, and provide constructive feedback to improve work conditions.
Upon receiving necessary training, the LPN may perform other duties as assigned, adhering to Wexford Health Sources, Inc. and facility regulations.
ENVIRONMENTAL CONDITIONS
The position is situated in a standard medical environment within a correctional facility, requiring daily interaction with inmates, correctional staff, and healthcare professionals. Temperature variations may occur while navigating both indoor and outdoor areas of the facility.
