Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at a long-term care facility in Claremont, NH. As a RN, you will be responsible for providing high-quality patient care, collaborating with the healthcare team, and promoting a positive and supportive environment for our residents.

Key Responsibilities
  • Observe and assess residents' physical and mental well-being, reporting any changes or concerns to the Supervisor or Director of Nursing.
  • Administer medications and treatments as ordered by the physician, accurately documenting all interactions in the MAR/TAR system.
  • Supervise Licensed Nursing Assistants (LNAs) in accordance with licensing requirements, assigning and measuring nursing staff resident care consistent with abilities and job description.
  • Accompany physicians on resident rounds to address medical needs and facilitate communication during 60-day reviews.
  • Administer oxygen and gastrostomy/jejunostomy feedings in accordance with facility guidelines and procedures.
  • Perform regular unit duties, including checking appointments, monitoring LNA documentation, and ensuring a clean and hazard-free environment.
  • Admit, discharge, and transfer residents, completing necessary documentation and notifications.
  • Complete 24-hour reports for assigned units, checking new orders and maintaining consistent communication with the Nurse Manager/Nursing Supervisor.
  • Communicate with residents' families to resolve concerns and problems, notifying them of new orders and obtaining consent when necessary.
  • Assume responsibility for unit management during shifts under the direction of the Nursing Supervisor or Director of Nursing.
  • Follow State and Federal Regulations regarding controlled medications, including counting at shift change and notifying Supervisors of discrepancies.
  • Assist with new nursing employee orientation, communicate continuing education needs, and deliver staff education and training as needed.
Requirements
  • 2+ years of current experience as a Registered Nurse.
  • Must be willing to float to different units.
  • IV certification preferred.
  • Previous travel experience and rural/small facility experience highly preferred.

  • 1 Supervisory Signed Reference from within the past two years of employment.
  • License/Certifications - Must be uploaded with profile (Current NH RN license, BLS and PERM ADDRESS).
Certification Requirements
  • BLS (AHA).
State License Requirements
  • New Hampshire.
Additional License Requirements
  • NH or Compact LPN license must be in-hand at submission. Facility will not pend.
Weekend Requirements
  • Every other weekend/Holiday coverage required.

  • None.
