Job Responsibilities

Develop and implement care plans based on comprehensive resident assessments, incorporating input from residents, families, and the care team.

Schedule PPS/MDS assessments for residents within established timeframes.

Lead the assessment coordination process to ensure accurate and timely completion of nursing sections.

Compile data for resident insurance updates and communicate effectively with therapy and insurance case managers to maximize reimbursement.

Oversee the MDS completion process, ensuring accurate and timely completion of all sections by responsible disciplines.

Qualifications

Must be a Registered Nurse currently licensed in the Commonwealth of Pennsylvania, with three years of experience in a similar role.

Two years of Long Term Care experience.

RAC-CTA certification preferred but not required.

Ability to follow and give accurate written and oral instructions, with visual acuity to effectively prepare and read reports.

Supervisory and leadership skills, with the ability to communicate effectively with staff, residents, and visitors.

Ability to coordinate and organize own work and the work of others to meet multiple job responsibilities and deadlines.

Proficient computer skills, with Microsoft Office experience preferred.
