About the RoleThis full-time position involves working with the homebound population, establishing strong relationships with patients and caregivers to assist them in reaching their maximum potential. As a Medical Home Health Case Manager, you will coordinate medical needs, utilize resources, and collaborate with the care team to ensure seamless care delivery.
Responsibilities- Assess, plan, implement, and evaluate nursing care for assigned patients in their own homes
- Develop and maintain a therapeutic relationship throughout the healthcare continuum
- Facilitate patient and family's right to receive quality, cost-effective care
- Collaborate with the care team to achieve positive patient outcomes
To be successful in this role, you should possess:
- A graduate degree from an accredited school of nursing
- At least one year of medical-surgical experience in an acute or subacute care facility
- A current Illinois RN license and CPR certification
- The ability to lift up to 35 pounds without assistance and assist with ambulation, including turning and transferring patients up to 200 pounds
We offer a competitive salary of $85,000 - $95,000 per annum, depending on experience, plus a range of benefits, including holiday pay, guaranteed hours, continuing education opportunities, and a comprehensive benefits package, including health, dental, vision, and life insurance. Additionally, you'll have access to pet insurance, wellness and fitness programs, mileage reimbursement, employee assistance programs, and a sign-on bonus.
