Job Summary

The LPN is responsible for administering skilled care to patients in their homes, adhering to state laws governing nursing practice, and collaborating with healthcare professionals to ensure seamless care delivery. Your primary focus will be on observing, recording, and reporting patient responses to treatment, as well as instructing family members in patient care and maintaining a healthy environment.

Key Responsibilities:
  • Deliver quality patient care in accordance with state laws and professional standards
  • Conduct home visits to assess patient needs and provide appropriate care
  • Communicate effectively with patients, families, and healthcare teams
  • Record and report patient progress and changes in condition
  • Instruct family members in patient care and maintain a safe environment
Requirements:
  • CURRENT LPN licensure in the state of practice
  • CURRENT Driver's License, vehicle insurance, and access to dependable transportation
  • CPR Certification required
