Key ResponsibilitiesThe Primary Care Physician (PCP) in our organization is expected to:
- Take ownership of patient outcomes: The PCP is accountable for delivering exceptional clinical care and ensuring cost-effectiveness for each patient within their panel of up to 450 patients. They recognize the importance of fostering a trusting relationship to positively influence patient outcomes.
- Act as a health coach: The PCP collaborates with patients to establish health goals, guiding them towards achieving these objectives while providing continuous support.
- Simplify processes for actionable results: The PCP prioritizes tasks effectively to facilitate behavior change for patients and to lead their care teams towards performance targets.
Our organization focuses on outcomes and value-based care. The PCP's performance is regularly assessed through various metrics, including patient admissions, appointment utilization, patient experience ratings, clinical gap closures, and medical cost measures. Each PCP will have specific goals related to these metrics and will work collaboratively with center and market leadership to achieve them. Cultivating a positive culture of compassion, accountability, and enthusiasm is essential, as PCPs are leaders within our centers.
Collaboration and ComplianceThe PCP must demonstrate the ability to work independently while also collaborating with other healthcare professionals. They will work closely with managers and medical directors to ensure adherence to guidelines and participate in risk and quality management initiatives.
Essential Duties- Provide comprehensive care for patients with acute and chronic conditions, particularly in the older adult population.
- Assume full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining engaged in the patient's ongoing care.
- Engage with hospitalists when patients are hospitalized, regardless of the hospitalist's affiliation.
- Conduct assessments, diagnoses, treatments, management, education, health promotion, care coordination, and documentation for patients with complex health needs.
- Lead a care team that includes medical assistants, care facilitators, and care coordinators.
- For patients unable to visit the office, collaborate with transitional care teams and other resources as needed.
- Participate actively in center management and assist in recruiting patients and providers.
- Perform additional duties as assigned by management.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty required.
- Current, active MD licensure in the state of employment is mandatory.
- A minimum of one year of clinical experience in a geriatric, adult, or family practice setting is preferred.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required.
- Basic Life Support (BLS) certification is required.
- A current DEA number for controlled substances is necessary.
