Position Overview

Independence Care System is seeking a dedicated and compassionate Registered Nurse to join our Care Management team as a Nurse Care Manager. As a key member of our team, you will play a vital role in collaborating with members to build a Person-Centered Service Plan (PCSP) that addresses their unique needs and goals.

Key Responsibilities

  • Conduct comprehensive in-home assessments to identify members' needs and develop personalized care plans.
  • Collaborate with members' healthcare providers to ensure seamless care coordination and effective communication.
  • Develop and implement Person-Centered Service Plans (PCSPs) that promote health, mobility, and community participation.
  • Participate in monitoring and updating member care plans in collaboration with Care Navigators and other team members.
  • Identify and develop necessary resources and services to support members' needs.
  • Maintain accurate and timely documentation of member interactions and care plans.

Requirements

  • Associate's Degree in Nursing (with extensive visiting nurse experience) or Bachelor's Degree in Nursing.
  • Current licensure as a Registered Nurse in New York State.
  • Minimum 1 year of nursing experience.
  • Experience in conducting and documenting assessment interviews.
  • Highly self-motivated with the ability to manage time independently.
