Job Summary

We are seeking a highly skilled and dedicated Speech-Language Pathologist to join our team at Creative Solutions in Healthcare - West. As a key member of our healthcare team, you will be responsible for providing high-quality speech and language therapy services to our residents.

Responsibilities
  • Evaluation and Assessment: Conduct thorough evaluations to identify residents' swallowing and cognitive deficits, and develop individualized treatment plans to address these needs.
  • Therapy Services: Provide skilled, medically necessary speech and language therapy assessments, as well as develop and implement individualized treatment plans to increase residents' independence and functioning.
  • Education and Training: Educate residents and caregivers on effective communication strategies and provide training on augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
  • Collaboration and Coordination: Collaborate with other healthcare professionals to coordinate diagnostic swallowing procedures (MBSS, FEES) as ordered by physicians, and provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
  • Documentation and Billing: Maintain accurate and timely billing records and supporting clinical documentation, and respond to family requests and concerns in a timely manner.
  • Quality Assurance: Ensure that residents' care is delivered in accordance with federal, state, and local regulations, and maintain confidentiality and safeguards for all patient information.
Requirements
  • Licensure and Certification: Active/Valid Texas license in Speech-Language Pathology, and good standing with all regulatory agencies and licensing boards.
  • Experience and Knowledge: SNF/Long term care experience preferred, current knowledge of treatment practices, and knowledge of Net Health Electronic Documentation System.
  • Personal Qualities: Ability to manage patients with different types of personalities, and a commitment to delivering high-quality care in a fast-paced environment.
