Your Responsibilities:

  • You will be responsible for coordinating and overseeing all aspects of patients' home care.
  • Collaborate with patient care teams to develop and implement care plans that meet the unique needs of each patient.
  • Educate patients and their families on condition, needs, and care.
  • Perform head-to-toe physical assessments and check vital signs as needed.
  • Administer medications and provide injections, IV therapy, or tube feedings as required.
  • Complete documentation timely and accurately.
  • Supervise LPNs and HHAs as assigned.
  • Regularly communicate patient progress to the clinical manager, physician, and care team.

Qualifications:

  • One year of clinical experience as a registered nurse (RN).
  • Current RN license, specific to the state(s) you are assigned to work.
  • Minimum six months home health experience.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Able to assess patient status and identify requirements relative to age-specific needs.
  • Familiarity with general computer use and functions.
