Travel Nurse RN Case Manager

About the Position
TravelNurseSource is working with TRS Healthcare to find a qualified Case Manager RN in a dynamic healthcare setting. We are seeking a Registered Nurse who is licensed to work in the state of South Dakota and has experience in Case Management.
Job Summary
The Registered Nurse assumes responsibility and accountability for incorporating the vision, values, mission, and critical goals of the organization into their job performance. This includes providing high-quality patient care, collaborating with healthcare teams, and ensuring seamless transitions of care.
Key Responsibilities
- Assume responsibility for patient care, including assessment, planning, implementation, and evaluation of care.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Ensure accurate and timely documentation of patient information.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions of care.
Requirements
- RN Registered Nurse License/Certification
- 2 years of recent experience as a Registered Nurse RN
- 1 year of recent experience specializing in Case Management
- Current Registered Nurse license to work in the state of SD
- Current Basic Life Support certification
