Job Title: Nurse Case Manager

Join the University of Maryland Medical System as a Nurse Case Manager and play a vital role in delivering high-quality patient care. As a key member of our team, you will be responsible for managing and overseeing the comprehensive assessment, planning, implementation, and evaluation of individual patient needs.

Key Responsibilities:
  • Conduct thorough assessments of patients' health needs and develop personalized care plans to ensure optimal outcomes.
  • Collaborate with healthcare providers, patients, and families to coordinate care and ensure seamless transitions between healthcare settings.
  • Identify and address potential health risks and barriers to care, and develop strategies to mitigate these issues.
  • Provide education and support to patients and families to promote health literacy and self-management skills.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed to ensure optimal patient outcomes.
Requirements:
  • Licensure as a Registered Nurse in the state of Maryland or eligibility to practice due to Compact state agreements.
  • 3-5 years of experience in care coordination or a related field.
  • Strong analytical, organizational, and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and projects.
