Job Summary

Polaris Pharmacy Services is seeking a highly skilled and experienced Staff Pharmacist to join our team. As a Staff Pharmacist, you will be responsible for the day-to-day operational duties associated with the dispensing of prescription medication in a long-term/post-acute care pharmacy setting.

Key Responsibilities
  • Verify the completeness and accuracy of all new and refill labels and orders.
  • Maintain resident databases to include current and past medical history, clinical and laboratory data, and concurrent medication.
  • Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances.
  • Provide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customers.
  • Document all communication with physicians, nurses, other members of the healthcare team, and patients/residents and their family members in the medical record.
Requirements
  • Current Pharmacist license active and in good standing in the state of practice.
  • Management experience preferred.
  • Bachelors degree or Pharmacy Doctorate from an accredited School of Pharmacy.
  • Ability to work flexible hours, including evenings, weekends, and holidays.
