Pharmacist - Home Infusion Therapist

We are seeking a highly skilled Pharmacist to join our team as a Home Infusion Therapist. As a key member of our Pharmacy team, you will be responsible for providing pharmaceutical care to patients in their homes, working closely with physicians, nurses, and other healthcare professionals to ensure the best possible outcomes.
Responsibilities- Evaluate and monitor drug therapy for home care patients, collaborating with healthcare professionals to optimize treatment plans.
- Adhere to organization policies, procedures, and regulatory requirements, ensuring compliance with state and federal laws governing pharmacy practice.
- Participate in quality assurance activities, contributing to the development and implementation of quality improvement initiatives.
- Coordinate new referrals and admissions, working with nurses, customer service, and distribution teams to ensure seamless patient care.
- Review, receive, and verify patient orders, documenting all necessary information.
- Assist physicians and other healthcare professionals with planning and implementing patient care, setting therapeutic objectives, and monitoring drug therapy outcomes.
- Conduct and document initial and ongoing assessments of patient, drug, and disease information to identify potential drug-related problems or needs.
- Monitor laboratory data, clinical data, and other patient information to evaluate the effectiveness of drug therapy.
- Provide consultation and answer inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use.
- Maintain thorough and accurate documentation in patient records, including all clinical activities and communication with healthcare professionals.
- Process and check medications, supplies, and equipment prior to dispensing from the pharmacy.
- Maintain clinical skills and knowledge of pharmacotherapy through ongoing education and professional development.
- Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
- Registered Pharmacist License in state(s) of practice.
- Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy.
- Experience with compounding infused drugs.
- Proven record of evaluating and monitoring drug therapies for home care patients.
- Good organizational and time-management skills.
- Participation and membership in professional societies and organizations is desirable.
- Excellent verbal and written communication skills.
- Flexible schedule with the ability to work on-call evenings and weekends as scheduled.
