Position Summary:

The Outpatient Physical Therapist (PT) is responsible for assessing and addressing patients' mobility and physical capabilities to enhance their functional performance.

Key Responsibilities:

Essential Duties:

  • Develop, oversee, and coordinate patient assessments and treatment plans tailored to specific age-related needs.
  • Conduct comprehensive evaluations, integrating data from medical records to assess functional status, physical impairments, movement issues, pain levels, community mobility, and rehabilitation potential.
  • Create individualized care plans based on evaluation findings, focusing on both short- and long-term objectives.
  • Adhere to established policies and procedures for documentation and record-keeping.
  • Engage in interdisciplinary meetings and collaborate effectively with team members to ensure seamless patient care.
  • Administer specialized treatments requiring advanced skills, emphasizing posture, movement, strength, endurance, cardiovascular health, coordination, joint mobility, flexibility, pain management, and recovery.
  • Evaluate patient responses to treatment and consult with the primary therapist regarding necessary adjustments to care plans.
  • Adapt tasks and modify the environment, tools, and treatment approaches to meet the unique needs of each patient.
  • Utilize resources efficiently and follow appropriate billing procedures.
  • Design personalized home exercise programs and instruct patients, families, and support staff on specific procedures and equipment use.
  • Schedule patient appointments based on workload and service demands, coordinating with other departments as needed.
  • Delegate treatment responsibilities to support staff according to their qualifications while maintaining overall accountability for patient care.
  • Ensure timely completion of required documentation and filing.
  • Perform any additional duties as assigned.

Education and Experience:

  • Current PT license or eligibility for licensure in the State of Oklahoma.
  • Current CPR certification.
  • Master's degree in a related field preferred.
  • Bachelor's degree in Science required.
  • Experience in rehabilitation preferred.
