Job Summary

The Registered Nurse is responsible for planning, organizing, and directing hospice care, with a focus on community health education and experience. This professional nurse builds on the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine hospice needs, providing a complete physical assessment and history of current and previous illnesses.
  • Provide professional nursing care by utilizing the nursing process, assessing and evaluating patient status, and participating in revising the plan of care as necessary.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed by the physician, and counsel patients and families on meeting nursing and related needs.
  • Act as a Case Manager, coordinating patient care for assigned caseloads, and communicate with the physician regarding patient needs and report changes in the patient's condition.
  • Teach patients and families/caregivers self-care techniques, provide medication, diet, and other instructions as ordered by the physician, and recognize and utilize opportunities for health counseling.
  • Provide and maintain a safe environment for patients, assist patients and families/caregivers and other team members in providing continuity of care, and work in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.
  • Attend interdisciplinary group meetings, participate in on-call duties as defined by the on-call policy, and ensure that arrangements for equipment and other necessary items and services are available.
  • Supervise ancillary personnel, delegate responsibilities when required, and assume responsibility for personal growth and development, maintaining and upgrading professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Requirements
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing.
  • Registered nurse with current licensure to practice professional nursing in the state, demonstrating excellent observation, problem-solving, verbal and written communications, and nursing skills per competency checklist.
  • Ability to organize and prioritize workload independently, maintain a current CPR certification, and have access to transportation for agency-related travel.
