Job Title: Licensed Master Social Worker

Job Summary: We are seeking a Licensed Master Social Worker to join our team. The successful candidate will be responsible for performing daily social work operations, including assessment, treatment, and coordination of care.

Responsibilities:

  • Conduct assessments of patients referred by Primary Care Providers using evidence-based assessment tools.
  • Develop and implement treatment plans using evidence-based approaches, including CBT, Motivational Interviewing, crisis intervention, and Interpersonal Psychotherapy.
  • Participate in collaborative care team meetings to make treatment recommendations.
  • Monitor treatment progress and coordinate care with the clinical team and health homes.
  • Provide crisis intervention services and risk assessments as needed.
  • Engage clients with limited utilization of behavioral health services through outreach efforts.
  • Document services in a timely manner.
  • Participate in weekly clinical supervision and team meetings as needed.

Qualifications:

  • LMSW licensure.
  • Master of Social Work degree from an accredited institution.
  • Ability to utilize behavioral health screening tools and perform biopsychosocial assessments.
  • Utilize DSM-V diagnostic criteria to diagnose and treat behavioral health conditions.
  • Assess for risk and collaborate with psychiatric and clinical teams around risk management.
  • Ability to utilize electronic health records for documentation of clinical services and review of relevant health data.
  • Communicate clinical information in a succinct and comprehensive way to integrated care teams.
  • Bilingual Spanish is strongly preferred.

Salary: $55,000-$65,000
