The Atrium Health Union West Transition Services and Internal Medicine clinic is an outpatient facility focused on assisting patients in reclaiming their physical and mental well-being. Our Transition Clinic operates as a multidisciplinary ambulatory Internal Medicine service, specifically designed for patients at elevated risk of complications and hospital readmissions due to medical intricacies or barriers to accessing healthcare. We provide integrated consultations involving a physician, pharmacist, and social worker. Throughout our 30-day follow-up period, we aim to identify and mitigate health barriers while emphasizing patient engagement, accessibility, and education. Care is delivered in-office, virtually, or with community paramedicine support tailored to each patient's requirements, alongside a traditional Internal Medicine clinic. Experience unparalleled clinical care, exceptional support, and a patient-first approach in an environment dedicated to wellness.
Key Features of the Role:
- 100% Outpatient care model
- Collaborative environment with 1 Physician and 1 Advanced Practice Provider
- Flexible office hours from Monday to Friday (7am-5pm)
- Shared call rotation, limited to phone coverage

- Board-Eligible or Board-Certified in Internal Medicine
