As a Housecalls Nurse Practitioner with UnitedHealth Group Inc, you will have the opportunity to provide high-quality, patient-centered care in the comfort of patients' homes. Our House Calls program is designed to deliver comprehensive clinical assessments and care management services to patients with complex needs.

Key Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.

Requirements:

  • Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in the state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and various dwelling conditions.
  • Reliable transportation to complete home visit assessments.

Preferred Qualifications:

  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • May be requested to obtain additional licensure in other geographic areas.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
  • Ability to travel throughout the state, beyond the assigned region, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
