**Home Health Care Opportunity**

Team Select Home Care is seeking a dedicated and compassionate Direct Care Worker to provide one-on-one in-home care to adult patients in the Limerick area.

**Key Responsibilities:**

  • Assist with daily living activities, including bathing, grooming, and toileting, as directed by the client's plan of care.
  • Provide support with daily tasks, such as feeding, meal preparation, and light housekeeping.
  • Assist with active and passive mobility work, as defined by the plan of care, and document any observed changes to the Director of Nursing.
  • Ensure timely and reliable transportation to doctor appointments and other scheduled activities.
  • Provide companionship and emotional support to clients.

**Requirements:**

  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift and transfer clients safely, with training provided for Hoyer lift use.

**Competitive Pay:** $15 per hour.
