Key Responsibilities:- Develop and supervise a team of RNs, social workers, and non-clinical support staff to deliver exceptional patient care.
- Evaluate and maintain case management skills and outcomes to ensure compliance with key model components.
- Manage all policies and procedures to ensure adherence to regulatory requirements and accreditation standards.
- Collaborate with the multidisciplinary team to ensure quality, timely, and cost-efficient care.
- Supervise and develop staff, plan, organize, direct, and evaluate to achieve program goals.
- Proficiency in data analytics and ability to work in a fast-paced environment.
- Minimum 5+ years of progressive management experience with demonstrated goal-oriented outcomes.
- Demonstrated leadership and change management experience.
- Case Management experience preferred.
- Strong understanding of clinical and financial outcomes.
- Ability to work in a fast-paced environment and demonstrate performance agility.
