About the Role

CenterWell Home Health is seeking a skilled Physical Therapist to join our team. As a Home Health Therapist, you will play a vital role in delivering high-quality patient care and rehabilitation services in the comfort of our patients' homes.

Key Responsibilities
  • Plan and administer prescribed physical therapy treatment and training for patients with various injuries, illnesses, and functional disabilities.
  • Test and screen patients' physical strengths to assist physicians in evaluating their level of function and develop effective treatment programs.
  • Develop and implement conditioning and rehabilitation programs in collaboration with the patient, physician, and clinical team.
  • Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program.
  • Supervise and evaluate the performance of physical therapy assistants and home health aides.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluations.

Requirements

  • Degree from an accredited Physical Therapy Program (approved by the APTA)
  • Minimum of one year physical therapy experience preferred
  • Current and unrestricted Physical Therapy license
