Key Responsibilities
  • Program Oversight: Provide strategic leadership and guidance to our care coordination team, ensuring that our program is aligned with our organizational goals and objectives.
  • Staff Supervision: Supervise and mentor a team of care coordinators, providing coaching and support to ensure that they have the skills and knowledge needed to deliver high-quality services.
  • Case Management: Oversee the management of complex cases, ensuring that our clients receive the support and services they need to achieve their goals.
  • Community Partnerships: Develop and maintain partnerships with community-based organizations, ensuring that our services are integrated with other community resources.
  • Quality Improvement: Identify areas for improvement and implement strategies to enhance the quality of our services.
Requirements
  • Education: Master's degree in a human services field or equivalent experience.
  • Experience: At least two years of experience in a human services position, with at least one year of experience working in community-based service provision and at least one year of experience working with children, youth, and families.
  • Certifications: Possess a LGPC or LMSW certification.
  • Skills: Strong interpersonal skills, ability to work effectively with diverse constituencies, effective verbal and written communication skills, and ability to analyze complex information and define and solve problems.

  • Competitive Salary: $52,000.00 - $65,000.00 per year.
