Nurse Practitioner

Westchester Jewish Community Services (WJCS) is seeking a skilled Registered Nurse to join our team in providing comprehensive nursing services to individuals residing in our homes. As a valued member of our team, you will collaborate with the interdisciplinary team and other health professionals to promote the optimal health and safety of each individual.
Responsibilities:- Coordinate mandated evaluations, such as physicals, dental, eye/vision, and audio evaluations.
- Provide follow-up for all medical recommendations, collaborating with the IDT to promote the strengths of each individual.
- Provide regular assessments to ensure the health and well-being of individuals.
- Collaborate with families and/or advocates as necessary regarding individuals' medical/health care needs.
- Participate with the IDT in evaluation of status and promotion of advanced directives and DNRs.
- Review immunization records for compliance with prescribed protocols.
- Administer immunizations and tests as required for staff and individuals.
- Ensure safe and accurate administration of medications.
- Teach Medication Course (AMAP) - Provide support training, evaluation, and encouragement to AMAP staff.
- New York State license to practice as a Registered Nurse required.
- Valid driver's license and ability to travel to various locations around Westchester.
- CPR Certification.
- Experience working with individuals who are developmentally or cognitively disabled.
- Must possess good interpersonal and organizational skills and be able to communicate effectively in English, both orally and in writing.
- Working knowledge of OPWDD regulations as it relates to nursing practices.
