Pediatrics and Internal Medicine Professional

As a Rural Medicine Specialist at Corewell Health, you will play a vital role in addressing the unique healthcare needs of rural communities. In this position, you will have the opportunity to work collaboratively with a dedicated team of healthcare professionals to provide high-quality, patient-centered care that addresses the complex health challenges faced by rural residents. From conducting home visits to coordinating care with specialists, your expertise and compassion will enable you to make a meaningful difference in the lives of patients and their families.
About the Role:
• Provide comprehensive medical care to rural patients, including those in underserved areas
• Conduct home visits and other outreach activities to reach patients who may face barriers to accessing care
• Collaborate with specialists and other healthcare professionals to ensure seamless patient care
• Stay up-to-date with the latest medical research, guidelines, and best practices to deliver evidence-based care
• Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal outcomes
• Maintain accurate and detailed patient records to support high-quality care and informed decision-making.
Responsibilities:
• Conduct initial assessments and develop treatment plans that address the unique needs of rural patients
• Provide ongoing care and management of chronic conditions, such as diabetes and hypertension
• Coordinate care with specialists, such as surgeons and cardiologists, to ensure seamless patient care
• Develop and implement health education programs to promote healthy habits and disease prevention among rural residents
• Partner with community organizations and local leaders to identify and address health disparities and other systemic issues affecting rural populations
• Participate in quality improvement initiatives to enhance patient care and outcomes in rural settings
• Maintain accurate and detailed patient records to support high-quality care and informed decision-making.
Requirements:
• Bachelor's Degree in a relevant field
• Board Eligibility or Certification in Family Medicine or Internal Medicine
• Licensure to practice medicine in the State of Michigan
• DEA Registration
• Basic Life Support (BLS) certification
• Ability to work collaboratively as part of a multidisciplinary team
• Strong communication and interpersonal skills
• Commitment to providing high-quality patient care and improving health outcomes
• Willingness to adapt to changing healthcare landscapes and technologies
• Ability to maintain confidentiality and handle sensitive patient information discreetly.
