Physical Therapist

CareerStaff Unlimited is seeking a skilled Physical Therapist to join our team in Unity, NH. As a valued member of our care team, you will play a crucial role in helping patients with medically complex conditions regain their quality of life.

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality patient care in a dynamic and supportive environment. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering exceptional patient outcomes.

• Provides Physical Therapy services to patients under the appropriate guidelines established by each individual client facility.
• Conducts screening of patients in accordance with the policies of the client facility to determine need for intervention/treatment.
• Evaluates patients to obtain data necessary for treatment planning and implementation.
• Develops treatment plans by identifying a problem list, including short and long-term goals and methods to achieve identified goals.
• Coordinates treatment plan with patient, family, and other team members.
• Implements and conducts treatment as outlined in treatment plan.
• Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
• Develops appropriate home or community programming to maintain and enhance the performance of the patient in his or her own environment.
• Documents results of patient's assessment, treatment, follow-up, and termination of services.

• Masters of Science or Doctorate in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• 1 year of verifiable, supervised professional experience.
• Current state licensure.
