Responsibilities:
- Apply a working knowledge of nursing theories and concepts to provide high-quality patient care.
- Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure seamless care delivery.
- Communicate effectively with patients, families, and healthcare team members to ensure clear understanding of patient needs and care plans.
- Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure high-quality patient care and outcomes.
- Collaborate with the Hospice team to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements:
- Current RN licensure in the state of practice.
- Successful completion of a nursing education from an approved school of nursing.
- At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
- Knowledge of the framework, function, and organization of a home health and hospice care program.
- Working knowledge of the principles and practice of nursing according to nursing practice act, agency policies and procedures, and reimbursement guidelines.
- 12 hours of continuing education annually.
- CPR certification.
- Ability to travel based on agency need, as well as weekend, evening, and holiday call assignments.
