Position: Licensed Practical Nurse (LPN) for Patient Assessments

Are you a skilled LPN with a passion for providing exceptional care in a home environment? MyHomecareJobs is seeking dedicated professionals to deliver a range of nursing services, including:

Key Responsibilities:
  • Conducting comprehensive Health Assessments
  • Scheduling and executing Supervisory Visits for other healthcare providers, including RNs, LPNs, CNAs, and PCAs

Care Coordination:

As an LPN, you will be responsible for delivering skilled nursing care to patients in their homes. You may also oversee both nursing and non-nursing care services as needed.

Your role will involve:

  • Identifying and assessing client needs
  • Delivering nursing services aligned with the established care plan
  • Preparing necessary equipment for treatments and procedures
  • Implementing infection control standards and preparing medications as necessary for patient safety
  • Making informed treatment recommendations based on patient acuity and potential for recovery
  • Excelling in the admissions process, including timely electronic documentation
  • Flexibility to manage admissions considering time and travel constraints

Documentation and Compliance:

This position requires accurate documentation of skilled care during all visits, adhering to agency standards. You will need to:

  • Follow regulatory guidelines and deadlines for clinical paperwork submission
  • Demonstrate proficiency in computer skills, including typing for reporting and updating documentation in online systems
  • Exhibit a willingness to work with electronic devices and telecommunication tools
  • Maintain organization and timeliness in clinical paperwork

Qualifications:

We are looking for candidates with excellent clinical skills, time management abilities, and strong communication skills, capable of conveying information clearly to both staff and the public, both verbally and in writing.
