Position Overview:
We are seeking a Full-Time Family Nurse Practitioner to deliver high-quality healthcare services. This role involves working collaboratively within our healthcare team to ensure comprehensive patient care at our health hub locations.

Key Responsibilities:
  • Deliver holistic, evidence-based care, including thorough assessments, accurate diagnoses, and effective treatment plans for a variety of health concerns.
  • Provide health education and counseling, focusing on preventive care and management of chronic conditions.
  • Administer immunizations and injections, ensuring patients receive appropriate vaccine counseling and education.

Qualifications:
Required:
  • A minimum of two years of relevant clinical experience.
  • Current certification in Basic Life Support (BLS).
  • Ability to communicate effectively in both English and Spanish is preferred.

Preferred:
  • Experience in urgent care, emergency care, or primary care settings.
  • Familiarity with electronic health record systems, particularly Epic.

Education:
  • Master's Degree in Nursing with a focus on Family Nurse Practitioner studies.
  • Current National Board Certification and a valid state license to practice as an Advanced Practice Nurse.
